Body release authorization

This form authorizes Texas Autopsy Services to release your loved one to the funeral home you have chosen once the examination is complete. Tell us which funeral home and where to send your copy, then review and sign. It only takes a minute, and the signed form is emailed to you, the funeral home, and our office.
